Position Summary
Under the direction of the Quality Improvement (QI) Manager, the Quality Improvement Specialist I supports and facilitates quality improvement initiatives for individual departments and across the organization. The scope of responsibility includes both clinical and operational settings. The QI Specialist I analyzes data, prepares reports, and develops tools to monitor performance and measure improvement. This role applies established quality improvement methodologies and techniques to support project implementation, performance monitoring, and continuous improvement efforts.
Quality Program & Measure Support
- Supports assigned quality initiatives through execution of defined tasks aligned with organizational priorities.
- Assists with implementation of standardized quality workflows across clinical and operational settings.
- Follows established quality protocols, specifications, and guidance provided by QI leadership, including technical guidance from the QI Specialist II.
- Participates in quality improvement activities, including PDSA cycles and workflow testing, as assigned.
- Conducts patient outreach via telephone or other approved communication methods to support appointment scheduling, rescheduling, and care gap closure, and documents interactions and outcomes in accordance with established workflows.
Data Preparation, Tracking & Validation
- Prepares and maintains quality trackers, patient lists, and logs using the EHR, Excel, and population health tools.
- Performs routine data validation and chart review to confirm documentation completeness and accuracy.
- Identifies data discrepancies or documentation gaps and escalates findings to the QI Specialist II or QI Manager.
- Supports care gap tracking and follow-up workflows without independently analyzing trends or outcomes.
- Maintains assigned quality trackers, patient lists, and logs using Excel, the EHR, and population health tools in accordance with established standards.
Audit & Documentation Support
- Assists with audit preparation by compiling documentation, reports, and supporting materials.
- Conducts chart reviews and documentation checks and supports audit readiness activities by validating required data elements and compiling and organizing supporting materials in accordance with established audit protocols.
- Supports supplemental data collection and submission preparation under direction of QI leadership.
- Ensures documentation activities align with approved standards and timelines.
Workflow Implementation & Clinic Support
- Supports implementation of approved workflows related to quality, care gap closure, and preventive care.
- Reinforces standardized processes with clinic staff as directed by QI leadership.
- Communicates workflow issues, barriers, or observations to the QI Specialist II or QI Manager.
- Provides logistical and operational support for quality initiatives across departments.
Collaboration & Escalation
- Collaborates with Quality, Informatics, Operations, and clinical teams to support assigned initiatives.
- Participates in QI meetings, huddles, and workgroups as assigned.
- Maintains compliance with HIPAA, confidentiality, and data integrity requirements in all activities.
- Escalates data discrepancies, documentation issues, or workflow barriers to the QI Specialist II or QI Manager following established workflows.
Required Qualifications
- High school diploma or equivalent required; associate’s degree or coursework in public health, healthcare administration, quality improvement, or a related field preferred.
- One to two years of experience in a healthcare, quality improvement, population health, administrative support, or patient-facing support role.
- Fluency in written and verbal Spanish.
- Basic to intermediate proficiency in Microsoft Excel, including use of spreadsheets for tracking, data entry, and routine reporting.
- Experience working with electronic data systems or databases and ability to document information accurately and consistently; experience with EHR systems preferred but not required.
- Ability to maintain confidentiality and comply with HIPAA and organizational privacy requirements.
Preferred Qualifications
- Experience supporting quality improvement initiatives, clinical quality measures, or audit preparation activities in an FQHC or community health setting.
- Familiarity with population health or quality management tools (e.g., care gap lists, registries, dashboards).
- Experience supporting care gap closure initiatives, preventive care outreach, or quality-related patient engagement efforts.
